29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions in the listing or labeling:    (a) Building 94, Machine Shop:  On or about January 20, 2015 and times prior thereto, metal receptacle boxes were not mounted on a recessed opening in a wall or permanently mounted to the wall.  The boxes were not designed and listed to be used as a pendant drop to provide temporary power. Employees were exposed to electric shock.  (b) Building 94, F/A 18 Overhaul and Repair Shop: On or about January 20, 2015 and times prior thereto, metal receptacle and circuit breaker boxes were not mounted on a recessed opening in a wall or permanently mounted to the wall.  The boxes were mounted to a job made portable yellow spider box. Employees were exposed to electric shock.

29 CFR 1910.334(a)(2)(ii): When there was a defect or evidence of damage that might expose an employee to injury, the defective or damaged item was not removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made:  (a) Building 94, Machine Shop:  On or about January 20, 2015 and times prior thereto, the 5/8 inch diameter flexible frayed and damaged electrical cord was not replaced to prevent possible contact with live parts. Volt Alert tester showed red light to indicate the cord was energized connected the Square D General Safety Switch. Employees were exposed to electric shock.
