29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  (a) Building 469, Machine Shop: On or about January 16, 2015 and times prior thereto, a Brilliant industrial abrasive chop saw, USN 001534, was not equipped with an enclosure guard for the rotating blade. Employees were exposed to coming in contact with the turning blade.

29 CFR 1910.212(b): Anchoring fixed machinery. Machines designed for a fixed location was not securely anchored to prevent walking or moving.  (a) Building 469, Machine Shop: On or about January 16, 2015 and times prior thereto, a Brilliant industrial abrasive chop saw, USN 001534, designed for a fixed location was not securely anchored to prevent walking or moving. The saw had pre-drilled holes cast into the base of the saw for attachment to the floor. Employees were exposed to coming in contact with the turning blade.

29 CFR 1910.213(p)(3): Each disk sanding machine did not have the guard, so arranged as to enclose the unused portion of revolving disk:  (a) Building 469, Machine Shop: On or about January 16, 2015 and times prior thereto, a Burr King 20 inch disc sander , USN 65888039722, did not have a guard to protect unused portion of the disc finishing machine. Employees were exposed to skin burn.

Sufficient access and working space was not provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.   (a) Building 469, Machine Shop: On or about January 16, 2015 and times prior thereto, a safety switch, 30 amp 600 Fed FMPNLDP, was not provided and maintained a minimum 3 feet working clearance.  Employees were exposed to electrical shock and burn.  (b) Building 469, Machine Shop: On or about January 16, 2015 and times prior thereto, a GE General Duty safety switch, 30 amp, 240 VAC, was not provided and maintained a minimum 3 feet working clearance.  Employees were exposed to electrical shock and burn.  (c) Building 469, Machine Shop: On or about January 16, 2015 and times prior thereto, a Square D safety disconnect switch was not provided and maintained a minimum 3 feet working clearance.  Employees were exposed to electrical shock and burn.
