29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  The employer did not ensure that Lockout procedures were developed for machines with more than one source energy and/or more than one hazard. Machines such as but not limited to the Turbo #1 and Turbo #2 require machine specific LO/TO Procedures.  Employees were exposed to a shock and/or struck-by hazard.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  The employer has not updated the hazard communication program and trained employees on the portion of the hazard communication program that explains the labels on shipping containers and the safety data sheets for items such as but not limited to gasoline, diesel fuel and lubricating oils including the order of information and how the employee could obtain and use the hazard information.
