29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Employees in the Pallet and Resaw Buildings perform servicing and maintenance activities on equipment including, but not limited to, the Turbo 505 Pallet machines and the Baker Resaw machines have multiple energy sources including pneumatic, hydraulic, and electric, which are exposing employees to unexpected energizing or start-up of equipment in that the current written machine specific energy control procedures for these machines were developed but did not identify all the potential energy sources.

29 CFR 1910.219(f)(3): Sprocket wheels and chains that were seven (7) or less feet from the floor or platform were not enclosed: Employees in the Pallet Building were exposed to in-going nip points. The employer did not provide a guard for a moving chain. The unguarded chain was discovered on the stacker side of the Turbo-505 pallet machine #1, about 4 feet 6 inches from the floor. Employees were permitted to walk and work near and around the unguarded rotating chain.
