OSH ACT of 1970 Section (5)(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from access onto elevated work platforms: a) On or about January 28, 2015 and times prior; proper access to elevated work areas: The employer failed to insure adequate climbing devices such as step ladder, extension ladder, or fixed/temporary steps were provided for employees to use for access onto the number 2 secondary centrifuge to conduct trouble shooting / repair of a hydraulic hose leak. The work area employees access is 4 feet 1.5 inches above the concrete floor enabling them to remove the hatch covers and inspect hydraulic lines.

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides and railings were not provided with a toe board where required: a) On or about January 28, 2015 and times prior; fall protection: The employer failed to insure fall protection was provided for employees while working from the elevated area 4 feet 1.5 inches above the concrete floor to conduct trouble shooting / repair of a hydraulic line leak on the #2 secondary centrifuge.
