29 CFR 1910.119(l)(1): The employer did not establish or implement written procedures to manage changes to process chemicals, technology, equipment, and procedures, and changes to facilities that affect a covered process. a) Nash Platform Area: On or about August 11, 2020 and at times prior to, the employer exposed employees to potential chlorine gas inhalation exposure in that the employer failed to ensure that a written management of change procedure was in place for a cam fitting that was installed for a maintenance activity.
