29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) Acid Separator Tank; On or about January 29, 2015 and at times prior thereto, the employer failed to enforce the use of respiratory and body protection when employees were exposed to respiratory and skin hazards from hazardous chemicals such as but not limited to chlorine gas and sulfuric during line breaking activities.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Acid Separator Tank; The employer failed to utilized the energy control procedures in that the employer failed to ensure employees entering the active zone wore the appropriate personal protective equipment for working in the active zone where hazardous chemicals such as chlorine gas and sulfuric acid were present.
