29 CFR 1910.24(h): Stair railings and handrails were not installed in accordance with the provisions of 1910.23:     a) Custom Plastic Specialties, stairways to the loft/ upper mezzanine areas and offices.  On or about January 29, 2015, the guardrail system on the stairways were not equipped with a mid-rail.  The stairways were utilized to access and egress the upper levels.

29 CFR 1910.37(b)(4): When the direction of travel to the exit or exit discharge is not immediately apparent, signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge:     a) Custom Plastic Specialties, mezzanine/loft.  On or about January 29, 2015, signs indicating the direction of exits were not installed in the loft area.  The exit route and to the nearest exit door was not readily apparent.

On or about January 29, 2015, exit signs were not illuminated.  These exit signs were equipped with internal illumination sources were not functioning.  The exit signs were located in areas of the building that included, but were not limited to:       (1) Exit door - employee main side entrance   (2) Exit door - west side of building   (3) Exit door - northeast side by eyewash station/stairs

The only procedure for the machine was a generic procedure that did not contain the specific methods, techniques, and procedural elements outlined in 29 CFR 1910.147(c)(4)(ii)(A) through 29 CFR 1910.147(c)(4)(ii)(D)

On or about January 29, 2015, employees conducted servicing and maintenance work on machines/equipment that included but was not limited to laminators, plastic blow molding machines, and bagging machines.  Annual periodic inspections of the lockout/tagout procedures (energy control procedures) had not been conducted.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, pinchpoints, and moving parts:     a) Custom Plastic Specialties, 5678 West Ridge Road Erie, PA, Blow Molding Department.  On or about January 29, 2015, employees were exposed to the hazards associated with unguarded moving parts of the HuiLong Blow Molding Press.  These moving parts included but were not limited to the moving arms and moving/indexing mold holder located inside the machine.  Employees were exposed to the moving parts when reaching inside the machine to place parts.     b) Custom Plastic Specialties, 5678 West Ridge Road Erie, PA, assembly area - bagger machine.  On or about January 29, 2015, employees were exposed to hand and finger injuries from the unguarded moving doors and sealing mechanisms on the bagger machine.
