The exposure level was derived from one sample collected over 14 minutes and assumed zero exposure for the one minute remaining in the STEL.  Employees who were exposed to formaldehyde at or above 0.1 parts per million were not included in a training program regarding formaldehyde exposure.     b.)    Custom Plastic Specialties, Inc., 5678 West Ridge Rd, Erie, PA 16506- Ink Room -On or about April 6th, 2015 and times prior thereto:  An employee (Ink room operator/mixer) who worked who mixed ink in the ink room was exposed to Formaldehyde for a Short Term Exposure Limit (STEL) of 0.1675 parts per million.  The exposure level was derived from one sample collected over 14 minutes and assumed zero exposure for the one minute remaining in the STEL.  Employees who were exposed to formaldehyde at or above 0.1 parts per million were not included in a training program regarding formaldehyde exposure.
