29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: A.) Custom Plastic Specialties, Inc., on or about June 9, 2017 and time prior thereto: An employee operating a Rennco bagging machine (M/N: 201, S/N: 2R4102884F0) suffered an amputation to the right ring finger when the employees finger was pinched in the heater bar. The Plexiglas guarding on the bagging machine was ineffective in that the guarding was large enough for the employee to stick his hand in the machine to perform work operations. B.) Custom Plastic Specialties, Inc., on or about June 9, 2017 and time prior thereto: Employees operate a Rennco bagging machine (M/N: 201, S/N: 2R4102884F0) to perform work operations including, but not limited to, bagging sunglasses. The opening where the plastic bagging film entered into the machine for the bagging operation was inadequate in that personnel could reach in over top the existing guarding. C.) Custom Plastic Specialties, Inc., on or about June 15, 2017 and time prior thereto: Employees operate a flamer machine that is equipped with a moving chain and sprocket conveying system. The return sprocket and its associated chain were not guarded.

Custom Plastic Specialties, Inc., on or about June 15, 2017 and time prior thereto: Employees did not have training on the proper selection and use of portable fire extinguishers upon initial employment at the establishment.
