29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) On or about 01-29-15, an employee was using an incorrectly installed personal fall arrest system which would have created a greater than six (6) ft. free fall. The worker was using a 4 foot lanyard and a Type II retractable (SRL) between his body harness and anchor while working on the edge of a roof approximately 36 ft. above the lower level.

One employee was not utilizing any fall protection equipment, one employee was wearing a body harness without attaching it to an anchored fall arrest system, and one employee had an incorrectly installed lifeline attached to an improperly donned body harness.

The W & L Insulation & Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 110201, citation number 01, item number 001 and was affirmed as a final order on 05-16-12, with respect to a workplace located at 635 Elmwood Avenue, Oshkosh, WI 54901.
