29 CFR 1910.95(g)(8)(ii)(B): Unless a physician determines that the standard threshold shift is not work-related or aggravated by occupational noise exposure, the employer did not ensure, when a standard threshold shift occurs, that for employees already using hearing protectors, employees be refitted and retrained in the use of hearing protectors and provided with hearing protectors offering greater attenuation if necessary:    a)  Throughout the Production Area - On or about May 7, 2015, the employer had not retrained or refitted employees in the use of hearing protection, or provided hearing protectors offering greater attenuation for five employees experiencing age-adjusted standard threshold shifts in one or both ears as documented from the March 26, 2015, annual audiogram test results.      Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.95(m)(2)(ii): The employee audiometric test records did not include at least the employee's name and job classification, the date of the audiogram, the audiogram, the examiner's name, the date of the last acoustic or exhaustive calibration of the audiometer, and the employee's noise exposure assessment result:    a)  Throughout the Production Area - On or about March 26, 2015, the employer did not include the employee's job classification at the time of the audiogram and most recent 8-hour time-weighted average noise exposure assessment in the employee's audiometric test record, which shall be retained for the duration of the affected employee's employment.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.1030(f)(2)(i): Hepatitis B vaccination was not made available within 10 working days of initial assignment to all employee(s) with occupational exposure:  a)  Throughout the Production Area - On or about February 3, 2015, the employer had not offered hepatitis B vaccinations within 10 days of initial assignment to, and key assignment for the emergency medical assistance first aid responders list where employees had a reasonable expectation of occupational exposure to blood while responding to calls from employees to unlock the locked first aid kits.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
