29 CFR 1926.501(b)(2)(ii): Each employee on a walking/working surface 6 feet (1.8 m) or more above a lower level where leading edges are under construction, but who was not engaged in the leading edge work, was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:  a) At the site, on February 5, 2015, the employer did not ensure employees performing decking work at height of eleven feet above the ground level were protected from fall hazards through the use of a personal fall arrest system, guardrail system, or safety net systems, exposing the employees to the hazards associated with falling.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  a) At the back of the house, on or about and at times prior to February 5, 2015, the employer did not ensure that the strain relief of a flexible cord of a "Bull" air compressor was intact and free of damage, exposing employees to the hazards associated with electrical shock and burns.
