29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   A) Building 3400, on or about June 22, 2016, employees were installing the roof sheathing (decking) on a building without the use of any fall protection equipment, exposing the employees to a fall hazard of up to 22 feet.    B) Building 3300, on or about June 22, 2016, employees were placing roof trusses on top of the framed walls, by standing on previously landed trusses that were lying on their side without the use of fall protection, exposing the employees to a fall hazard of 9 feet.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:   A) Building 3400, on or about June 22, 2016, the wooden job made ladder used by the employees to gain access to the roof did not extend three feet above the roof edge, exposing employees to a fall hazard of approximately 9 feet.

29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969 Safety Standards for Powered Industrial Trucks:   A) By building 3300, on or about June 22, 2016, the employer was operating a Lull rough terrain forklift that did not have a functioning horn, amber strobe light, or back-up alarm as prohibited by, Section 7, Paragraph 702 (I), exposing employees to a struck-by hazard.
