29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level: a) On February 11, 2015, on the east side of the house, no fall protection provided for employees on 11 foot, 11 inch high platform on a two tier, prefabricated metal frame, mobile scaffold platform.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: a) On February 11, 2015, on the east side of the house, ladders or stairs were not provided to access a second level balcony. Employees climbed up a two tier, prefabricated metal frame, mobile scaffold to a 11 foot, 11 inch high platform, and then climbed over an 42 inch high railing to the balcony.
