29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: a) On February 11, 2015, a 120 volt outlet receptacle used by to power a portable sander was pulled out of the wall box, exposing employees to electrical and fire hazards.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement: a) On February 11, 2015, on the perimeter of the house, approximately 100 3/8 inch diameter rebar that protruded about 24 to 30 inches from concrete masonry units were not guarded against impalement hazards. Jianhua Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.701(b), which was contained in OSHA inspection number 948908, citation number 1, item number 2 and was affirmed as a final order on January, 22, 2014, with respect to a workplace located Lot 23-1-D, Agana Heights, GU 96919.
