29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.    Roof - south side: Eye protection was not used when an employee used a portable demolition hammer to chip concrete. Employee was exposed to eye and struck-by hazards.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    Fronting Quichocho Street: A fiberglass stepladder that did not extend at least 3 feet above the landing was used to access the roof, nor was there a grab rail at the top of the landing. Employees were exposed to fall hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At the South Side, Atop the Roof: Two employees engaged in form and masonry activities were not protected from falling to the lower level.  Jianhua Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 977503, citation number 1, item number 2 and was affirmed as a final order on August 27, 2014, with respect to a workplace located at LOT 25-1-R3NEW-R1, Fonte Dr. Agana Heights, GU 96910.
