On or about February 11, 2015 the employer did not ensure that flexible cables were secured so that strain relief would be provided on a 220v roller in building 1 by column 217 so as to prevent accidental employee conduct. The failure of the employer to ensure that strain relief was provided exposed employees to electrical current.
