29 CFR 1910.134(g)(2)(ii): The employer did not ensure that employees leave the respirator use area to replace necessary air cartridge components of the respirator.    During confined space entry into the "Dip Coat Tank" performed on or about the week of February 4, 2015 employees were exposed to asphyxiation hazards in that the air supply ran out and was changed 4 times. The instructions and equipment given to employees did not ensure that employees had sufficient time to exit before the air ran out.

29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146    Employees entering permit required confined spaces were exposed to asphyxiation hazards in that a written permit space entry program was not developed and implemented to comply with 29 CFR 1910.146:    a) On or about February 4, 2015 chemical hazards were not identified and evaluated before employees entered the "Dip Coat Tank".    b) On or about February 4, 2015 ventilating equipment required to obtain acceptable entry conditions in the "Dip Coat Tank" was not provided.    c) On or about February 4, 2015 employees did not use the supplied air respirator when working near the top of the "Dip Coat Tank".    d) On or about February 4, 2015 entry operations did not include testing conditions to ensure that acceptable entry conditions exist in the "Dip Coat Tank".    e) On or about February 4, 2015 entry into the "Dip Coat Tank" was authorized without filling out an entry permit documenting the completion of appropriate measures.    f) On or about February 4, 2015 training of employees was not sufficient to establish proficiency in the procedures for entering the "Dip Coat Tank".    g) On or about February 4, 2015 authorized entrants for the "Dip Coat Tank" were not made aware of the specific hazards they could be facing during entry, including signs symptoms and consequences of exposure.    h) On or about February 4, 2015 attendants for the "Dip Coat Tank" were not made aware of the specific hazards the entrant could be facing during entry, including signs symptoms and consequences of exposure.    i) Entry permits did not identify the hazards of the permit space.    j) Entry permits did not identify rescue and emergency services.    k) Entry permits did not identify the communication procedure to be used by authorized entrants and attendants.    l) The training records did not include the signature or initials of the trainer.
