29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources:    (a) Mill 5, Moulding Department - The employer did not provide enough lockout locks to authorized employees.  The employer provided only one lock each to the operators of the Weinig Hydromat 22B CNC moulding machine.  These operators performed servicing and maintenance work, including, but not limited to, setting up and clearing jams, on the Weinig Hydromat 22B CNC moulding machine, which had multiple energy sources including pneumatic and electrical, exposing employees to the hazards such as contacting moving cutter heads and in-going nip points, on or about February 23, 2015.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:    (a) Mill 5, Moulding Department - Operators and the supervisor performed servicing and maintenance work, including, but not limited to, setting up, on the Weinig Hydromat 22B CNC moulding machine, which had multiple energy sources including pneumatic and electric.  Each operator had only one lockout/tagout lock and the supervisor carried one lockout/tagout lock, so when two employees worked together to do servicing and maintenance on the Weinig Hydromat 22B CNC moulding machine, one employee would lockout/tagout the air and one employee would lockout/tagout the electric rather than both employees both applying lockout/tagout to both energy sources, exposing employees to the hazards such as contacting moving cutter heads and in-going nip points, on or about February 9, 2015.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:  (a) Mill 5, Moulding Department - The electrical safety interlock switch on the guard (hood) on the cutting heads of the Weinig Hydromat 22B CNC moulding machine no longer functioned to prevent the operation of the cutting heads with the hood open, exposing employees to the hazard of contacting the knives of the rotating cutting head, on or about February 9, 2015.   No abatement certification or documentation required.

29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA:    (a) Weaber, Inc., 1231 Mount Wilson Road, Lebanon, Pennsylvania - An employee suffered an amputation to the top of the left little finger on February 9, 2015 as a result of contacting the R4 cutting head on the Weinig Hydromat 22B CNC moulding machine while clearing a jam.  The employer failed to report this work-related amputation to OSHA within 24 hours.  (The employer reported this amputation to OSHA on February 17, 2015.)    Corrected during the inspection.
