The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. (a) Building 306, Machine Shop: On or about January 15, 2015 and times prior thereto, Chicago Steel Press Brake hydraulic, Size 334, Serial Number 3145, was not provided with an awareness barricade or barrier guard behind the press brake. Employees were exposed to amputation related injuries.
