29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels :  Reefer Flat:  On or about February 9, 2015 and times prior thereto, employees were exposed to over 97 decibels on a daily basis and were not receiving annual hearing tests.  This exposed employees to occupational noise exposure hazard without follow-up evaluations to test for hearing loss.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  Factory:  On or about February 9, 2015 and times prior thereto, employees were using super red and harbor master without an eyewash in the area.  This exposed employees to eye injuries from not having suitable flushing capabilities for immediate use.

29 CFR 1910.1200(h)(3): The employee training did not include the requirements of 29 CFR 1910.1200(h)(3)(i) through (h)(3)(iv):  (a)  Vessel employees:  On or about February 4, 2015 and times prior thereto, employees had not been trained on the requirements of the Global Harmonization System.  The lack of training would limit their ability to understand the new requirements of the program.
