Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse: At the time of inspection, the employer failed to ensure that the storage of material did not create a hazard. a) The racking system, at product K151 A and K021 A, has been damaged; b) The pallets on the top level of the racks, were overhanging over the edge of the racking system.

Employees were riding in a basket that was on the forks of the powered industrial vehicles to access higher levels of the racking systems.
