29 CFR 1910.178(m)(2): No person shall be allowed to stand or pass under the elevated portion of any truck, whether loaded or empty. a) On or about August 5, 2025, employees engaged in the disassembly of a racking system passed beneath the elevated portion of the VIFT FB20. The employer did not ensure to take the necessary precautions to ensure that employees disassembling the racking system remained clear of the area under the elevated section of the truck.

29 CFR 1910.178(m)(4): The employer shall prohibit arms or legs from being placed between the uprights of the mast or outside the running lines of the truck. a) On or about August 5, 2025, an employee involved in the disassembly of a racking system inadvertently placed their foot between the uprights of the mast of the VIFT FB20. The employer failed to implement appropriate safeguards and/or procedures to prohibit employees disassembling the racking system from placing their feet in this hazardous area, resulting in a serious amputation injury.
