29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Finishing Department, Slitter #3 - On or about March 8, 2015 an employee(s) operating Slitter #3 was caught by the unguarded in-going nip point(s) between the take-up rewind roller and the adjacent inboard tensioner roller. (b) Finishing Department, Slitter #1 - On or about March 16, 2015 employee(s) operating Slitter #1 were exposed to being caught by the unguarded in-going nip point(s) between the take-up rewind roller and the adjacent inboard tensioner roller. (c) Finishing Department, Slitter #4 - On or about March 18, 2015 employee(s) operating Slitter #4 were exposed to being caught by the insufficiently guarded in-going nip point(s) between the take-up rewind roller and the adjacent tensioner roller.
