The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  a. On or about March 6, 2020, and at times thereto, employees operating the Jagenberg Slitter machine were exposed to three unguarded nip points at the slitter heads located approximately twenty eight inches from the floor, thereby exposing employees to a caught in hazard.

29 CFR 1910.219(c)(2)(i): All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires.  a. On or about March 6, 2020, and at times thereto, employees operating Laminator 6 were exposed to unguarded rewind shaft approximately four foot from the floor while reaching in the machine to clean rollers, thereby exposing employees to a caught in hazard.

On or about March 6, 2020, and at times thereto, employees operating Laminator 4 were exposed to an unguarded protruding shaft end on the Foil Unwind shaft and Paper Unwind shaft.      b. On or about March 6, 2020, and at times thereto, employees operating the Johnstown Slitter were exposed to an unguarded protruding shaft end on the bottom roller of the Web Guide section.      c. On or about March 6, 2020, and at times thereto, employees operating the Laminator 7 were exposed to an unguarded shaft end on Foil Unwind 1 shaft.

Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.   a. On or about March 5, 2020, and at times thereto, the employer did not ensure that sufficient access was provided to the 120/240 volt panel and 480 volt transformer disconnect due to chopper equipment located below within the safe working space of the electrical equipment.      b. On or about March 6, 2020, and at times thereto, the employer did not ensure that sufficient access was provided to the 480 volt dust collector disconnect switch due to the placement of a trashcan within the safe working space of the disconnect.     c. On or about March 6, 2020, and at times thereto, thee employer did not ensure that sufficient access was provided to the 480 volt battery charger disconnect switch located at the dust collection system.

Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.   a. On or about March 6, 2020, and at times thereto, employees removing unlaminated foil from inside the Laminator 7 line were exposed to a caught in hazard due to an unguarded ingoing nip point where material entered between two rollers to the coating station area, thereby exposing employees to a caught in hazard.
