29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) was not done in the sequence as required by 29 CFR 1910.147(d)(1) through d(6):  a.  On or about March 2, 2024, employees were exposed to machine hazards associated with moving parts and in-going nip points during servicing and cleaning of the coating station rollers and doctor blade of Laminator 2, LAMN-102.  The employer failed to implement energy control application steps as LOTO devices were not applied to the energy isolating devices per the employer's procedure 21.207 L2-LOTO Procedures, and 1910.147(d)(4) requirements.  As a result, the remaining applicable energy control elements, involving dissipation of residual energy, (d)(5)(i), and verification of isolation, (d)(6), were not implemented to protect employees from machine servicing hazards.  b.  On or about March 2, 2024, employees were exposed to machine hazards associated with moving parts and in-going nip points during servicing and cleaning of the coating station rollers of Laminator 2, LAMN-102.  The employer failed to implement energy control application steps as the machine waws not shut off nor turned off to perform the servicing work, per the 1910.147(d)(4) requirements.  As a result, the remaining applicable energy control elements, involving machine isolation, (d)(3), LOTO device application, (d)(4), dissipation of residual energy, (d)(5)(i), and verification of isolation, (d)(6), were not implemented to protect employees from machine servicing hazards.  International Converter LLC was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1910.147(d), which was contained in OSHA inspection number 1613563, citation number 1, item number 3 and was affirmed as a final order on December 2, 2022, with respect to a workplace located at 17153 Industrial Hwy, Caldwell, OH 43724.

29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.   a.  On or about March 3, 2024, employees were not protected from hazards, including an ingoing nip point, with effective machine guarding designed to prevent the employees from contacting the moving parts of the Laminator during production activities.  Employees performed normal production activities, including quality assurance checking and adjustments of material passing through the rollers on the rear side of the Laminator 2/LAMN-102 coating station, with the machine's physical barrier guard removed and safety switches bypassed with tape.  International Converter LLC was previously cited  for a violation of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1467533, citation number 1, item number 5 and was affirmed as a final order on July 31, 2020, with respect to a workplace located at 17153 Industrial Highway, Caldwell, OH 43724.
