29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment:    A personal protective equipment hazard assessment was not performed to determine the type of personal protective equipment needed such as chemical goggles, face shields, chemical aprons, and etc., for employees performing maintenance on equipment, changing propane cylinders on fork trucks, and etc.   Employees working in the warehouse and production area were exposed to chemical burns, lacerations, and other health hazards.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    The employer's lock out / tag out program did not include completely filled out machine specific procedures for all hardwired equipment with multiple energy sources.  Employees performing maintenance and preventive maintenance in the production areas were exposed to caught between, struck by, and electrical hazards.
