29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer is failing to protect employees from amputation hazards while working on a truss machine. This was most recently documented at 11200 N 148th St, Waverly, Nebraska on July 16, 2018 when employees were observed assembling wood truss structures in an Alpine Floor Master Machine that had no guarding to protect against an amputation hazard. On July 12, 2018 an employee received an amputation injury to his left thumb while assembling a truss on the Alpine Floor Master.

This was most recently observed at 11200 N 148th St, Waverly, Nebraska on July 23, 2018 when an electrical power cord was observed with its outer insulation pulled away and interior wires were visible. The end of the cord was covered in saw dust.
