This was most recently documented at 11200 N 48th St, Waverley, Nebraska on May 29, 2019 where employees who operate the Automatic Omni Saw were exposed to a projecting rotating shaft that was unguarded.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: The employer is failing to protect employees from injuries from contact with unguarded sprocket wheels and chains. This was most recently documented at 11200 N 48th St, Waverley, Nebraska on May 29, 2019 when employees operating an Automatic Omni Saw were exposed to an unguarded chain and sprocket mechanism. On May 14, 2019 an employee received an amputation injury after contacting the unguarded chain and sprocket of the Omni Saw.
