29 CFR 1910.178(m)(8): There was not sufficient headroom under overhead installations, lights, pipes, sprinkler systems, etc.: On March 25, 2015, an employee was burned while operating the Hyster Forklift, Model Number 60FT, Serial Number N177V02245M, when the mast of the forklift struck an electrical bus bar causing the bus bar to break, creating arching and the metal to melt that fell on the employee seated in the forklift.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: On March 25, 2015, an employee was burned while stacking metal frames with a forklift when the mast of the forklift struck an electrical bus bar causing melted metal to fall on the employee and the employer did not report the injury until March 30, 2015.
