29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    On or about April 6, 2015, the employer did not ensure procedures for the control of hazardous energy clearly and specifically outlined the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy.  The procedures did not include steps for shutting down and securing machines and equipment, steps for the placement and removal of lockout devices, and requirements for the testing and verification of the effectiveness of the energy control measures. The employer operates mechanical presses, broaching machines, lathes, CNC machines, cranes, hoist, saws, and other machines used in the manufacturing of musical instruments. The employer did not ensure employees performing service, maintenance, setups, and unjamming parts on mechanical presses, broaching machines, lathes, CNC machines, cranes, hoist, saws, and other machines used in the manufacturing of musical instruments were trained to understand the purpose and function of the energy control program and had the knowledge and skills required for the safe applications, usage, and removal of energy control devices.  Energy sources include, but are not limited to; electrical, pneumatic, natural gas, mechanical, and hydraulic power.
