29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): 1) On April 8, 2015, and at prior times, employees operating a band saw in the machine shop were exposed to amputation hazards when cutting metal, plastic, and other materials. The employer did not ensure that operators adjusted the guard to the size of stock being cut in order to cover the unused portion of the saw blade.
