29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded:  a) Machine Shop: On or about May 20, 2019, and at times prior, workers were exposed to caught in hazards at the point of operation of a Fadel VMC 4020, Model 906-1, S/N 9404062, due to the front sliding door interlock not functioning due to cut switch conductors.       b) Machine Shop: On or about May 20, 2019, and at times prior, workers were exposed to caught in hazards at the Fadel VMC 4020, Model 906-1, S/N 9504496, due to the front sliding door interlock not functioning due to a missing electromagnetic switch.    c) Machine Shop: On or about May 20, 2019, and at times prior, workers were exposed to caught in hazards at the Fadel Model VMC-3016L, S/N 012001122946, due to the front sliding door interlock not functioning due to programming changes that prevented proper function of the switch. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Machine Shop: On or about May 20, 2019, and at times prior, workers were exposed to caught in hazards while operating any of four Bridgeport milling machines, S/Ns J177799, J298885, 275736, and J214158, without a shield or other protective device to protect workers from the point of operation during milling operations.  b) Machine Shop: On or about May 20, 2019, and at times prior, workers were exposed to caught in hazards while operating the Bridgeport milling machine, S/N J177799 while the left and right guards protecting the belt and cone pulley nip points were not installed.  Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).
