This condition exposed employees to laceration and amputation hazards.  b) Mid-West Machine Products, Inc.: On or before May 25, 2022, the employer did not protect employees from injury in that the Accurpress 725014 250 ton press brake was used to bend narrow workpieces where employees gripped the workpiece and operated the press brake with their hands 4 inches or less from the dies.
