29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Mid-West Machine Products, Inc.: On or before May 25, 2022, the employer did not protect employees from injury in that the Hyd-Mech S20A horizontal bandsaw was not equipped with a guard over the nonworking portion of the blade. Approximately 6 inches of blade was exposed outside the clamping area. This condition exposed employees to laceration and amputation hazards.  b) Mid-West Machine Products, Inc.: On or before May 25, 2022, the employer did not protect employees from injury in that the Accurpress 725014 250 ton press brake was used to bend narrow workpieces where employees gripped the workpiece and operated the press brake with their hands 4 inches or less from the dies. This condition exposed employees to crushing and amputation hazards.  Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR  1910.213(c)(1):Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:  a) Mid-West Machine Products, Inc.: On or before May 25, 2022, the employer did not protect employees from injury in that the Delta Unisaw 10 inch tablesaw was not guarded with an automatically adjusting hood. The hood was broken and not an effective guard. This condition exposed employees to amputation hazards. 29 CFR  1910.213(c)(2):Hand-fed circular ripsaw(s) were not furnished with a spreader to prevent material from squeezing the saw or being thrown back on the operator:   a) Mid-West Machine Products, Inc.: On or before May 25, 2022, the employer did not protect employees from injury in that the Delta Unisaw 10 inch tablesaw was not equipped with a spreader to prevent kickback hazards. Employees make narrow cuts for crating and other purposes in the shop. This condition exposed employees to kickback hazards. 29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded:  a) Mid-West Machine Products, Inc.: On or before May 25, 2022, workers were exposed to caught in and amputation hazards at the point of operation of a Fadel VMC 4020, Model 906-1, S/N 9404062, due to the front sliding door interlock not functioning.       b) Mid-West Machine Products, Inc.: On or before May 25, 2022, workers were exposed to caught in and amputation hazards at the point of operation of a Fadel VMC 4020, Model 906-1, S/N 9504496, due to the front sliding door interlock not functioning.    c) Mid-West Machine Products, Inc.: On or before May 25, 2022, workers were exposed to caught in and amputation hazards at the point of operation of a Fadel Model VMC-3016L, S/N 012001122946, due to the front sliding door interlock not functioning. Mid-West Machine Products, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1403290, citation number 1, item number 1a, and was affirmed as a final order on 7/3/2019, with respect to a workplace located at 6255 Joyce Drive Golden, CO 80403.

29 CFR  1904.41(a)(2): 29 CFR 1904.41(a)(2) Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. If your establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee.
