On the roof of a residential property, eye protection was not provided to employees operating a Super Roofer Max CN445R2 nail gun, exposing the employees to a struck-by hazard, on or about 4/15/2015.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:  a. At the front entrance of the home, the portable extension ladder used to access the roof was not secured from displacement, exposing employees to the hazard of falling approximately 10 feet, on or about 4/15/2015.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On the 2nd story 5-12 pitch roof, employees installing shingles were not protected from falling approximately 25 feet, on or about 4/15/2015.   b. On rear patio roof, an employee installing shingles was not protected from falling approximately 10 feet, on or about 4/15/2015.   Brad McDonald Roofing & Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1004881, citation number 1, item number 1 and was affirmed as a final order on 12/11/2014, with respect to a workplace located at 7053 Peregrina Loop, Wesley Chapel, FL, 33545.
