29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Machine Shop Area - On or about April 16, 2015, an employee was exposed to being struck-by metal shavings and caught-in hazards while operating a Pratt & Whitney lathe without a guard protecting the employee from rotating parts and projectiles.      b) Machine Shop Area - On or about April 2, 2015, an employee was exposed to being struck-by metal shavings and caught-in hazards while operating an Enco (Model #: 510-2585) lathe without a guard protecting the employee from rotating parts and projectiles.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:    a) Metal Fabrication Area - On or prior to April 16, 2015, two extension cords were plugged into junction boxes that were being used as permanent wiring to power two DeWalt chop saws.    b) Assembly Area - On or prior to April 16, 2015, an extension cord plugged into a junction box was being used as permanent wiring to power a riveter. 29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    a) Metal Fabrication Area - On or prior to April 16, 2015, an extension cord with visible insulation damage to the cord was in use powering a DeWalt chop saw.    b) Press Brake Area - On or about April 16, 2015, a foot pedal with visible insulation damage to the cord was in use on the Cincinnati 90-350 Proform Hydraulic Press Brake (Serial No. 52930).
