29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: a) Facility - On or prior to June 27, 2016, employees were exposed to caught-in hazards when clearing jams on the Cincinnati Shear (20012 SHEAR NO. 42647). The employees did not utilize lockout/tagout when clearing jams and did not understand when the energy control procedure needed to be utilized. The company did not provide training to authorized employees on lockout/tagout.
