29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: Biggs Terminal: On or about April 22, 2015 and at times prior thereto, fit-testing was conducted where employees had facial hair that came between the sealing surface of the facepiece and the face. This condition impacted the fit of the respirator, affected the facepiece seal, and exposed employees to inhalation hazards.

29 CFR 1910.307(c)(2)(i): Equipment was not approved for the class of location and for the ignitable or combustible properties of the specific gas, vapor, dust, or fiber that would be present: Biggs Terminal, Tool Room: On or about April 22, 2015 and at times prior thereto, 120 volt light switches and outlets were not approved for the location. This condition exposed employees to fire and smoke inhalation hazards.
