29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times.  Old Administration Building:  On or about 29 April 2015 and times prior thereto, the emergency lighting in the room adjacent to the heater room was inoperative.  This condition exposed employees to delays in existing the vessel resulting in smoke inhalation and fire hazards.

29 CFR 1910.110(f)(2)(i): Containers in storage shall be located so as to minimize exposure to excessive temperature rise, physical damage, or tampering by unauthorized persons.  Outside Rec Cache building:  On or about 29 April 2015 and times prior thereto, three 5-gallon propane tanks were stored outside in close proximity to vehicles and trailers.  this condition exposed employees to a fire and burn hazard.

(a) Shop Two:  On or about 29 April 2015 and times prior thereto, the 120 volt electrical circuit Breaker panel was blocked.  This condition exposed employees to an electrical shock and burn hazard.  (a) Old Admin Building, room next to heater room:  On or about 29 April 2015 and times prior thereto, the 120 volt electrical circuit Breaker panel was blocked.  This condition exposed employees to an electrical shock and burn hazard.

Old Admin Building, Heating control room:  On or about 29 April 2015 and times prior thereto, the oil burner emergency switch cover was removed exposing electrical wiring.  This condition exposed employees to an electrical shock and burn hazard.

(a) Shop 2:  On or about 29 April 2015 and times prior thereto, the 120 volt electrical outlet adjacent to the battery charging station had a broken face plate.  this condition exposed employees to an electrical shock and burn hazard.  (b)  Old Amin Building:  On or about 29 April 2015 and times prior thereto, the electrical light switch in the room adjacent to the heater room has a broken switch.  This condition exposed employees to an electrical shock and burn hazard.
