29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. Garage bay and Woodworking shop areas; on or about 29 April 2015 and times prior thereto; the employer did not ensure that the work areas were clean and orderly. Two instances were noted: a. In the garage bay area; numerous items were loosely scattered on the floor impeding the walking and working surface. b. In the woodworking shop; numerous items were loosely scattered on the floor and along the hall corridor leading to the East exit; boxes, tires and various other items were being stored. This condition exposed employees to a tripping hazard.

Shop Two, West doorway; on or about 29 April 2015 and times prior thereto; the west door exit did not have an exit sign posted. This condition exposed employees to delays in exiting the building and smoke inhalation, fire, and tripping hazards.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use. Two instances were noted where suitable quick drenching and flushing of the eyes and body were not readily available: a. Shop Two; on or about 29 April 2015 and times prior thereto; in the battery charging area; no station was immediately available. b. Garage bay; on or about 29 April 2015 and times prior thereto; the employer did not ensure that suitable facilities were immediately available for quick drenching and flushing of the eyes and body. A ladder was stored in front of the eyewash station where battery charging is conducted. This condition exposed employees to eye trauma hazards.

29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical. Woodworking shop; on or about 29 April 2015 and times prior thereto; the employer did not ensure that the work areas were clean and orderly. Safety data sheets for each hazardous chemical located in the workplace were not available. A random sample of chemicals were selected. Three instances were noted: (a) Sunnyside, Brush Cleaner, 16 ounce. (b) Plastic Wood Cellulose Fiber Filler 1.875 ounce. (c) Touch n Foam Home Seal, Expanding Sealant 16 ounce. This condition exposed employees to chemical burns, irritation of the eyes and upper respiratory tract, and skin dermatitis hazards.

Throughout the Field Office facilities; on or about 29 April 2015 and times prior thereto; portable fire extinguishers were not visually inspected monthly. Some extinguisher are located outside and are exposed to extreme environmental elements. This condition exposed employees to delays in exiting the building and smoke inhalation, and fire hazards.
