29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about 4/30/2015, at the above addressed jobsite, the employer did not ensure employees exposed to a fall hazard while conducting roofing activity were adequately trained in fall hazards and prevention.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    On or about 4/30/2015, at the above addressed jobsite, employees installing tiles on a 4:12 roof pitch were exposed to a fall hazard of approximately 20 feet above ground level due to lack of fall protection being used.    CJM Roofing, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 801621, citation number 01, item number 001, issued on 3/1/2013, with respect to a workplace located at 8158 Solana Drive, in the city of Cooper City, Florida. The final order date is 3/7/2013.

29 CFR 1926.502(d)(6)(i): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself:  On or about 4/30/2015, at the above addressed jobsite, an employee was exposed to a fall hazard of approximately 24 feet above ground level while installing tiles on a 4:12 roof pitch due to the snaphook of the lanyard being attached to a knot of a lifeline.
