Condition noted on or about 04/28/15.     b- West Side Of Site, 217 Mantoloking Road, Brick NJ: Employees were exposed to fall hazards in excess of 19 feet to the ground surface below when utilizing an aluminum extension ladder (to access and egress the roof of a residential structure) that contained visibly bent side rails of the ladder's fly section.  The employees using this ladder had not received any ladder hazard identification training from the company. Condition noted on or about 04/28/15.
