These instances included but are not limited to the following:       a. Coupler for Center Pump Motor in Anhydrous Cooling Tower Pumping Station,   b. Coupler for West Pump Motor in Anhydrous Cooling Tower Pumping Station,   c. Coupler for East Pump Motor in Anhydrous Cooling Tower Pumping Station.   d. Couplers on 2 pump motors in Fibrous Mother Liquid. and  e. Coupler on pump motor near Door 26 in Acid Recovery.

Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer did not label each container of hazardous chemicals in the workplace:       Viscofan USA, Inc. had employees exposed to chemical hazards due to hazardous chemical containers not being labeled, including but not limited to: West Coal Filter, West Anhydrous C, East Anhydrous C, and Glycerin (south Glycerin, 14T027).  This labeling would include the following:      a. Product identifier and   b. Pictures, symbols, or combination thereof which would provide employees information regarding the hazards of chemicals.

29 CFR 1910.22(a)(2): Where wet processes were used, drainage was not maintained, and false floors, platforms, mats, or other dry standing places were not provided when practicable:    On or about May 11, 2015, Viscofan USA, Inc. had employees exposed to slip and fall hazards while walking down Extrusion Aisle, due to the accumulation of water in the aisle.           Viscofan USA, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1910.22(a)(2), which was contained in OSHA inspection number 980809, citation number 1, item number 1 and was affirmed as a final order on November 26, 2014, with respect to a workplace located at 915 N. Michigan Avenue, Danville, IL.
