29 CFR 1910.219(f)(1): Gear(s) were not guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii): On or about February 16, 2022, employees working in the Cellulose Extrusion Department were exposed to amputation and entanglement hazards because: a. the drive gears located in Bay 1, powering the PM1, PM3 and "black strip" pumps were not guarded or enclosed in accordance with the standard. b. the drive gears located in Bay 3, powering the PM7 pumps were not guarded or enclosed in accordance with the standard.
