29 CFR 1926.51(f)(1): The employer did not provide adequate washing facilities for employees engaged in the application of paints, coatings, herbicides, or insecticides, or in other operations where contaminants may be harmful to employees. Such facilities were not in near proximity to the worksite and were not equipped as to enable employees to remove such substances: a) On or about 05-14-15 during construction of a bridge/street roundabout, employees were spreading and finishing wet concrete formulated with Portland Cement without any soap available on site for washing.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided: a) On or about 05-14-15 during work on a new bridge/street roundabout, employees had provided their own work gloves which were not impervious to water for work with wet concrete formulated with Portland Cement. No company gloves available on site. b) On or about 05-14-15 during work on a new bridge/street roundabout, three employees were working in wet concrete formulated with Portland cement with exposed skin on legs and/or arms. All three employees were wearing shorts, one employee was wearing a sleeveless shirt, and one employee was wearing a short sleeved shirt.
