29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about May 20, 2015 the employer did not ensure the polyplanker hot air laminator line #2 was properly guarded to prevent employees exposures to in-going nip points while hand feeding foams sheets through the top rollers of the polyplanker laminator. b) On or about May 20, 2015 the employer did not ensure that the rotating shaft of the rip saw on the left side of the line #1 Polyplanker hot air laminator was properly guarded. Employees were exposed to caught in hazards during set up procedures where employees must come within close proximity to the rotating shaft as they feed foam through the rip saw. IVEX Protective Packaging Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1) which was contained in OSHA Inspection Number 850603, citation number 1, item number 21/a and was affirmed as a final order on August 12, 2013, with respect to a workplace located at 456 South Stolle Drive in Sidney, Ohio 45365.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) IVEX Protective Packaging, Sidney, Ohio; On or about May 20, 2015, the employer did not ensure that employees were protected from the point of operation when operating the line #1 Polyplanker hot air laminator. Employees feeding foam sheet into the rip saws during set-up activities were exposed to injuries such as but not limited to amputations and lacerations. b) IVEX Protective Packaging, Sidney, Ohio; On or about May 20, 2015, the employer did not ensure that employees were protected from the point of operation when operating the line #1 Polyplanker hot air laminator. Employees placed beverages and tools on the metal foam guide located within approximately one inch of the automated cross cut blade. IVEX Protective Packaging Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(3)(ii) which was contained in OSHA Inspection Number 850603, citation number 1, item number 21/b and was affirmed as a final order on August 12, 2013, with respect to a workplace located at 456 South Stolle Drive in Sidney, Ohio 45365.
