29 CFR 1910.269(c): The employee in charge did not conduct an adequate job briefing with the employees involved before they started each job, including at least the following subjects: hazards associated with the job, work procedures involved, special precautions, energy source controls, and personal protective equipment requirements:    Jobsite located at 158 Cronin Avenue, Forestville, Connecticut:  On, or about, May 4, 2015, a work crew was assigned to the Forestville substation to replace three bushings on an oil filled circuit breaker, 12A-2T-2.  The work involved removed the oil from the tank enclosures, locking out the interrupter mechanism and entering a permit required confined space.  The employer did not conduct an adequate job briefing prior to dispatching the crew and did not provide enough information to the crew to conduct adequate Tailboard job briefings on site.      A.  The employer did not provide adequate work procedures to properly lockout the circuit breaker energy sources.  The correct Instruction Book was not provided and the proper method to isolate the energy sources was not reviewed.  Also, testing procedures after the replacement was complete were not reviewed.    B.  The employer did not review and/or ensure that employees understood the company's confined space procedures, including informing employees that the space was a permit required confined space, procedures to be followed to evaluate the space and the proper documentation that needed to be completed and retained.

29 CFR 1910.146(c)(7)(iii): The employer did not document the basis for determining that all hazards in a permit space had been eliminated, through a written certification that contained the date, the location of the space, and the signature of the person making the determination:    Jobsite located at 158 Cronin Avenue, Forestville, Connecticut:  On, or about, May 5, 2015, employees were directed to work inside of a circuit breaker tank enclosure to replace bushings.  The entry was made as a non-permit confined space entry once the atmosphere was tested and the interrupter mechanism was lockout out.  The interior configuration of the tank including component obstructions and the slope of the floor as well as the presence of oil on all the exposed surfaces was not evaluated and addressed as potential hazards.  The employer did not document the basis for determining that all hazards in the space had been eliminated before entering under a non-permit classification.

The permit(s) issued for entry into the confined space was not retained for at least one year following the completion of the job.
