29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:  Ref. CGA P-1-2006, Section 5.7.4 as referenced by CGA P-1-1965, Section 3.3.1.      Substation Operations Building, Aisle near Building Entrance:  On, or about, December 12, 2018, two compressed air cylinders that contained sulfur hexafluoride (SF6) were stored, unsecured, next to a fence and storage cabinet.

29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel: or the distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:      On, or about, December 12, 2018, in the following locations off-hand grinders were not provided with proper tongue guards:     A.  Line Cage:  A Milwaukee bench mounted grinder was not provided with a tongue guard on the right side.      B.  Fleet Garage, Tire Room:  The tongue guard provided on the Blue-Point pedestal mounted grinder was not adjusted within one quarter of an inch from the wheel and the tongue guard was not installed perpendicular to the wheel.    C.  Substation Operations Building, Maintenance Shop:  The tongue guard provided on the left side of the Millers Falls bench grinder was adjusted over the outer guard and not in position to offer protection from wheel breakage.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910:       A.  Fleet Garage, Electrical Room:  On, or about, December 12, 2018, a refrigerator was located across from Panel A-4, a General Electric 480/277 volt panelboard.  The working space had been reduced to 37 inches.      B.  Substation Operations Building, Maintenance Shop:  On, or about, December 12, 2018, a workbench and battery charger were located in the working space for Panel S9, a General Electric 120/208 volt panelboard.

Fleet Garage, Electrical Room:  On, or about, December 20, 2018, items such as a cardboard box, sign holders and angled material were stored on the floor in the working space of Panel 5-6, a General Electric 480/277 volt panelboard.

29 CFR 1910.304(b)(4)(ii)(B): Where connected to a branch circuit supplying two or more receptacles or outlets, a receptacle may not supply a total cord- and plug-connected load in excess of the maximum specified in Table S-4:    Substation Operations Building, Electricians Day Room:  On, or about, December 12, 2018, a GE refrigerator, GE microwave oven and a Keurig coffee maker were energized through a Belkin 15 ampere relocatable power tap.  Both the microwave oven and coffee maker exceeded the 12 ampere (80% load) permitted on the power tap.
