29 CFR 1910.269(o)(2)(i): The employer did not establish and enforce work practices for the protection of each worker from the hazards of high-voltage or high-power testing at all test areas, temporary and permanent: Jobsite located at 300 Wethersfield Avenue, Hartford, Connecticut: On, or about, June 13, 2015, the employer had not established and enforced work practices to protect each worker involving in testing operations related to a cable fault repair. An employee was subjected to an arc flash incident when he attempted to reapply a protective ground after removing a test lead from a phase in pad mounted switchgear. The cable that was being tested re-energized due to a failure in a vacuum switch.
